To make the most of your visit with Dr. Maria, there are a few steps you should take before each appointment. The better prepared you are, the more effective your visit will be.
For your initial appointment, please arrive 10-15 minutes prior to your scheduled appointment time to fill out the appropriate forms. The forms are also available below for your convenience. Please ensure the forms are completed in their entirety to avoid any delays.

What Do I Need To Bring?
For IV THERAPY/CHELATION & NATUROPATHIC MEDICINE appointments:
- Photo ID
- Bloodwork/Lab Work completed within the last 4 months
- If you do not have this, Dr. Maria can provide a script for you to obtain the bloodwork, which may incur additional fees based on your testing location.
- All lab tests are the responsibility of the client and the results should be directly sent to Dr. Maria Scunziano-Singh at the WellCome OM Integral Healing & Education Center, LLC.
- Please note that we cannot schedule your follow-up appointment until the bloodwork is received.
- Any additional medical records or consultation reports
- A list of all medications & natural supplements you are currently taking (you may also bring them with you, if desired)
- Intake Form Packet (download with link below)
For NUTRITIONAL COUNSELING appointments:

Why Do I Need An Initial Consultation For Most Appointments?
An initial consultation is a critical component of your treatment plan. An initial consultation may include, but is not limited to:
- Review of your medical history
- Identification of your health goals and experiences
- Comprehensive health examination and findings (for IV Therapy and Naturopathic Medical Therapy)
- Education for process and possible causes of the suspected disease condition
- Development of a treatment plan
Please note that we do not offer same-day IV therapy. An initial consultation is required for the reasons listed above.
